Our Fees . . .
Counseling sessions normally last about an hour; psychiatric
sessions, about a half hour. Charges are per session, not per hour.
However, some initial psychiatric evaluations may take longer and
be subject to a higher charge. Effective October 1, 2008,

- Individual, Couple and Family: $110 per session
Psychiatric: $120 per session
Group: $ 45 per member
Insurance, Payment Plans and Discounts. . .
Some counselors take insurance. If you are unable to use
insurance, payment plans may be arranged. This normally consists
of spreading the cost of 6 to 12 sessions over a six month period.
Charges and payments are made monthly so that the total amount
owed is paid in six months
You may qualify for a discount based on your income. The
difference between the discounted fee and the standard fee is
covered by the Azimuth Client Assistance Fund. This fund is
supported by Azimuth donors who realize the importance of our
counseling and want to make it affordable to all.
Clients applying for a discount must complete the Application for
Reduced Fee form and submit verification of their annual income
(a copy of a recent pay stub or previous year's tax return showing
Adjusted Gross Income). Clients will be notified of the decision by
their therapist. Any questions regarding the decision will be
referred to the Bookkeeping Department. All financial information
will be kept confidential. To insure this confidentiality, clients will
be requested to provide the last 4 digits of their social security
number to gain access to this information.

of the 2009 Federal Poverty Guidelines*) will be used to determine
eligibility for a discounted fee:
